I don't know of a specific rule or guideline in CMOS, but in reading your examples, I understand the meaning without the numbers being italicized.  In other words, I don't think you need to italicize any of the numbers. ~ Mark

As long as there's no cause for misreading, there's no reason both numbers can't be spelled out. Use the most common form for the numbers, though. In this instance, "He cut three 2x4s" might be a better (or at least alternative) way to state this.
