I'd leave it at "twelve 9s." The "twelve" is being used as an actual number, and the "9" is referring to the digit, which the reader just saw in its digit form. It is clearer this way.

According to Wikipedia, they're octal modes. https://en.wikipedia.org/wiki/Chmod#Octal_modes So I think "six-digit octal modes" or "six-digit octal permissions" would be fine. But I also think r, w, and x worked well enough in their time :) I just rm -rf problems like this.
