I just had a client contact Simon and Schuster regarding a book proposal. The website told her to submit a Title Information Sheet. Neither she nor I can find the term anywhere. Can anyone tell me what that is? Is it part of the front matter or something else entirely?

Artisan, a division of Workman Publishing, does gorgeous cookbooks. Chronicle Books, here in San Francisco, also does some beautiful ones. It's worth going to the bookstore and finding cookbooks you like and then seeing who publishes them! Good luck!

Working in the publishing field, I'd hazard a guess that most publishers of fiction follow CMOS. But with fiction being artistic and creative rather than technical, there's no rigid way to approach it. There are many variations on the theme.
