I agree with UTK's solution. When two numbers are adjacent, it always makes it more readable if one is spelled out and the other is a numeral.

I agree with Grumps on both the hyphenation and on spelling out the numbers. There's no reason (that I'm aware of, since I don't know the context) to use numerals for these small numbers.

I concur with what's been said so far. Don't spell out phone numbers. It's too clumsy to read and, well, just too strange.
