The 3rd is the classical approach. However, in modern fiction you may find authors skipping entirely the quotes and relying on capitalization to indicate the start of the quoted thought. Thus the 1st could be seen.

Oh my God ... oh, my god ... Oh God ... God! ... Dear god ... I'm on the hunt for guidance about the capitalization of God in exclamations. In 13.39, Faltering or interrupted speech, I find an example that keeps "god" uppercase (without a comma): “The ship . . . oh my God! . . . it’s sinking!”
